What Questions Should I Ask When Choosing a Rehab Facility?

I get calls from families all across Tennessee who are trying to figure out where to send someone they love. They have usually done some research online, looked at a few websites, and still have no idea how to tell the difference between a good facility and one that just looks good. So I put together the questions I would want anyone to ask, including us. Any facility worth choosing should be able to answer all of these without hesitation.

My name is Nick Talmo. I am the Executive Director of Willow Grove Recovery in Palmyra, Tennessee, and I am a person in long-term recovery. I have been on both sides of this conversation.

Are You Licensed and What Are Your Credentials?

This is non-negotiable. Any legitimate facility should be licensed by their state’s department of mental health or substance abuse services. In Tennessee that means TDMHSAS. Beyond state licensure, ask whether they are federally recognized as a provider and what specialties they are registered for.

Why does this matter? Because federal recognition determines which insurance plans they can bill. A facility that cannot answer this question clearly either is not credentialed properly or does not want you to know. At Willow Grove we are federally registered across four specialties including substance abuse rehabilitation and adult mental health, which means we are equipped to treat dual diagnosis, where addiction and mental health conditions exist together. Most people who come to us have both, and a facility that can only treat one side is not going to get to the root of the problem.

What Does a Typical Day Look Like?

Ask for a real rundown. What time does the day start? When are group sessions? Is there free time, and how much? What happens in the evenings? Structure is one of the most important parts of early recovery, and a good facility should be able to tell you exactly what the days look like without being vague about it.

Red flag: if the answer is mostly about amenities and not about clinical programming, keep asking.

What Treatment Approaches Do You Use?

There is no single method that works for everyone. A quality facility should use a combination of evidence-based approaches and be able to explain them in plain language. Common ones include the Twelve Step model, cognitive behavioral therapy, group therapy, individual counseling, and mindfulness-based practices. Ask whether treatment is individualized or whether everyone follows the same program.

Do You Treat Co-Occurring Mental Health Conditions?

This is one of the most important questions and most people do not think to ask it. Research consistently shows that a large majority of people with addiction also have an underlying mental health condition, whether that is depression, anxiety, trauma, or something else. If a facility only treats the addiction and ignores the mental health piece, the underlying issue stays unaddressed and the risk of relapse stays high.

Ask specifically: do you have licensed mental health clinicians on staff? Can you treat depression, anxiety, or trauma alongside the addiction? A yes to both is what you are looking for.

How Is Family Involved?

Addiction affects the whole family, and recovery goes better when family is part of the process. Ask what family involvement looks like. Are family members kept informed? Is there family therapy or education available? What does the facility do to help families understand what their person has been going through?

A facility that treats the person in isolation and sends them home without preparing the family around them is setting everyone up for a harder road.

What Is the Staff to Client Ratio?

More staff means more attention. Ask how many clients each counselor carries and how many clinical staff are on site at any given time. This question often separates facilities that are genuinely invested in outcomes from ones that are running a high volume operation. There is no universal magic number, but if the answer is vague or the ratio seems high, push for clarity.

What Happens After Treatment?

The day someone walks out the door is one of the most vulnerable moments in recovery. Ask what the transition plan looks like. Is there a step-down to outpatient care? Is there an alumni program? Does the facility help with next steps or just discharge people and wish them well?

Aftercare is not an add-on. It is part of treatment. A facility that does not take it seriously is not thinking about long-term outcomes.

Do You Accept My Insurance?

Always ask this before you go too far down the road with any facility. Get the specifics. Not just yes or no, but what your out of pocket will look like and what the billing process involves. A good admissions team will walk you through this honestly and completely before you make any decisions.

What Does It Feel Like to Call Them?

This is the one question you answer yourself. When you call, notice how they treat you. Are they warm? Do they listen? Do they answer your questions without rushing you toward a decision? That first call is a preview of what the experience inside will be like.
